When a lip is cleft, the obicularis muscle is unattached where the cleft is. Ordinarily the obicularis looks like a tire that goes around the opening of the mouth. When it is reattached, the place where it was put back together is going to be weaker than the rest of that very powerful muscle. So, when you smile afterward, that side will be more "lazy" than the other.

I think I have never seen a more beautiful smile than Jacob's crooked little smile. I'm totally in love with it, and I think it adds a beautiful character to his face.

When he smiles, the sun comes out to kiss it, it is that beautiful. And it's also crooked.
