Information we're asking for includes:

1. Family Name

2. Names & Birth Dates (particularly birth dates of the children, adults don't really need to add this information if you don't want to!)

3. Identify child/person with cleft, and type of cleft.

4. State/Country in which you live

5. Email address*

* If you want to protect your privacy, get a free email address from places like (just to name a few): Hotmail, Yahoo, Excite. There are so many places you can get a free email address from; just do a search on the internet. *

If you are a household of one - just send your own picture. Also, you don't really need to use a studio portrait for your block, just a clear picture of yourself and/or your family :-)
